See if you qualify

Answer a few short questions about your health on your phone or computer. It takes about two minutes. We will tell you right away whether your condition qualifies.

Pick a time that works for you

If you qualify, you choose an appointment time. Most appointments are available within three days. You put a card on file to hold your spot, but you are not charged until just before your visit.

You only pay if you qualify and book a visit.

Fill out a short form

Before your visit, you fill out a brief medical history form at your own pace. It takes about five to ten minutes. Your information is kept private and secure.

Have a short video visit

At your appointment time, you click a link we email you. It works on any phone, tablet, or computer. There is no app to download. You will see your doctor on the screen. They review your information, ask a few questions, and complete your medical certification. The visit is about five minutes.

Get your signed paperwork

If the doctor certifies your condition during the visit, your signed Florida form is ready in your secure online account, usually within 24 hours. You can download it and print it.

We can mail it to you if printing is difficult.

Take it to your county office

Bring the signed form, along with a copy of your driver's license, to your county tax collector office to get your placard. Many counties also let you mail it in. Provider Notes does not issue the placard itself; your county does that final step.

This is the last step. Then you have your placard.

What if I am not good with computers?

That is completely fine. The steps are simple, and the video visit works with a single click, no app needed. If you get stuck at any point, you can call us and a real person will help you. A family member can also do this with you or for you.

Do I have to pay just to find out if I qualify?

No. Checking if you qualify is completely free and takes about two minutes. You are never asked for a credit card to check. You only pay the $119 fee if you qualify and decide to book a visit.

Is my personal information safe?

Yes. Your health information is protected and kept private under federal HIPAA law, the same rules that protect your information at any doctor's office. We do not sell or share your information.

Is this a real doctor?

Yes. Your visit is with a real physician who is licensed by the State of Florida. You can look up their license yourself; we list their license numbers right on our website.

What if I do not qualify?

If your condition does not qualify, you will find out during the free eligibility check, before you pay anything. You are never charged for checking.
