If a heart, lung, neurological, or vision condition limits your mobility, a Florida-licensed physician can evaluate you by private video visit and, if you qualify, complete Florida's HSMV 83039 form. You submit the signed form to your county tax collector’s office or a license plate (tag) agency, who issues your placard. $119 one-time fee. Free eligibility check first, no card required.

Who this is for

You may be eligible to book an evaluation if any of the following apply:

  • Cardiac conditions that limit how far you can walk
  • Lung or respiratory conditions such as COPD or oxygen dependence
  • Neurological conditions that affect walking or balance
  • Vision impairment affecting safe walking, or an inability to walk 200 feet without resting

Is approval guaranteed if I pay?+

Do I have to be in Florida?+

Yes. You must be physically located in Florida during your video visit. The permit is a Florida permit.

Do I need to send my medical records ahead of time?+

No. Your doctor reviews your condition with you during the video visit. If anything else is needed, they will tell you.

Is this a substitute for seeing my regular doctor?+

No. This is a one-time evaluation for a parking permit only. It does not replace primary or ongoing care.

How much does it cost?+

A flat $119, charged after your visit. The eligibility check beforehand is free.
