No. The 2-minute eligibility check is free and does not ask for a credit card. You are only asked to book and pay if your condition falls within our scope of practice. If we are not a fit, you'll be told before any payment.

Nothing is charged when you book. Your card is saved and charged $119 twenty-four hours before your appointment. Cancel or reschedule before then and you are not charged at all. After that the fee is non-refundable, except in the limited cases in our Refund Policy.

The $119 pays for the doctor's clinical evaluation, not the outcome, the same as any doctor's office. Our doctors are paid the same whether they certify you or not, so you get an honest opinion, not an approval mill. Most patients who pass the free eligibility check qualify at the visit.

You may qualify for a full refund if it's rejected specifically because our certification was insufficient. Email help@providernotes.com within 15 days with written proof. Administrative or patient-error rejections are not refundable.

Florida law permits licensed physicians and chiropractors to evaluate patients by telehealth and to complete disabled parking permit certifications within their scope of practice. Both of our providers are Florida-licensed, and you can verify their licenses yourself on our Our Doctors page.

One of two Florida-licensed doctors. Dr. Dean Fishman, a chiropractor, handles orthopedic and musculoskeletal conditions like back pain, joints, arthritis, and post-surgical recovery. Dr. Barry Schapiro, a physician, evaluates walking limitations caused by medical conditions, including cardiac, lung, neurological and vision-related cases, plus surgical recovery and pregnancy. He assesses how the condition affects your ability to walk; he does not treat or manage the condition itself. We match you with the right doctor based on your condition, so you don't have to choose.

Florida-qualifying conditions include severe arthritis, multiple sclerosis and other neurological conditions, post-stroke mobility limits, post-surgical recovery, herniated discs, cardiac conditions that limit walking (Class III or IV), lung disease or oxygen dependence, vision impairment affecting safe walking, and needing a cane, walker, or wheelchair. In each case the limitation has to be severe enough that walking 200 feet without stopping to rest is a problem. Our chiropractor and physician together cover the full range.

The free check filters out cases clearly outside our scope before booking. During the visit, the doctor's clinical judgment is the final determination. If your condition doesn't meet Florida's criteria, you'll get a clear explanation.

They take a history and apply clinical judgment to it, which is how functional mobility is assessed in an office too. The doctor asks how far you get before resting, what devices you use and how much they help, whether you use oxygen, what your diagnosis is and how it affects an ordinary day. They also observe what video shows: how you move, any device in use, your breathing. Where the picture is unclear they may ask for records. The decision is the doctor's, not yours, which is why certification is not guaranteed.

The eligibility check takes about two minutes. You pick from the next open times, often same day or next day, and the visit itself runs about five minutes. Your signed form is emailed within 24 hours of the visit if the doctor certifies you. After that, timing is up to your county tax collector, which varies by county and by whether you go in person or mail it.

No referral, and no records required. The doctor evaluates how your condition affects your ability to walk safely, which is what Florida's form actually asks about. If you have records and want to share them, you can, and they may help for less visible conditions. Most patients do not need them.

Florida requires a Florida driver license or Florida ID card number on the application, so you need one of the two. If you cannot get to a driver license office because of your condition, Florida has a provision for a doctor to certify that. The visit is straightforward. We email you a secure link, and you tap it from your phone, tablet, or computer, with no app to download. Yes. A caretaker can complete the check and book using the patient's information. The patient does need to be on the video visit, since Florida law requires the doctor to examine them directly. The caretaker is welcome to join.

Yes, and it uses the same certification. Florida issues a wheelchair symbol license plate to anyone who qualifies for a disabled parking permit, using form HSMV 83039 or HSMV 83007. It is for long-term limitations only, costs $28 plus registration fees, renews annually, and is issued in place of a permanent placard rather than in addition to it. For most people a placard is more flexible and free.

Yes. We are a HIPAA covered entity, your visit takes place on a HIPAA-compliant platform, and every vendor that touches your information has a business associate agreement in place. A permanent placard is valid for 4 years, is issued free by the state, and is for long-term disabilities. A temporary placard is valid for 6 months, has a $15 state fee at the county, and is for conditions expected to improve.

Yes. Florida law requires the placard to be issued by the Department of Highway Safety and Motor Vehicles through your county tax collector or tag agency. We complete the medical certification by video only; you take or mail the signed form to your county with your driver's license. Most accept it by mail.

Florida disabled parking placards are generally honored in other U.S. states under reciprocity agreements. Rules vary, so check with the state you're traveling to for any local requirements.
