Website Privacy Policy

Effective October 28, 2026. Access Path Management, LLC, doing business as Provider Notes.

This policy covers this website only.

Your health information is covered by our Notice of Privacy Practices, which is a different document with stronger protections. Once you begin an evaluation request, that notice governs.

1. What this site collects

This site is informational. It has no forms, no accounts and no payment. When you choose to check your eligibility you leave this site for secure.providernotes.com, which is the booking application and is covered by the Notice of Privacy Practices.

What this site does collect:

  • Standard server logs, kept by our hosting provider: the address of the page requested, the time, and a truncated network address.
  • Analytics, if enabled, in aggregate and without anything that identifies you individually.

2. What this site does not collect

  • Your name, unless you send it to us in an email
  • Anything about your health
  • Payment details of any kind

3. Cookies and advertising measurement

This site sets no advertising cookies and no cross-site tracking.

4. If you email or call us

We keep what you send so we can answer it. Please do not include health information in an email; email is not a secure channel and we will ask you to continue by another route if you do.

5. Children

This site is not directed at anybody under 18 and we do not knowingly collect information from them.

6. Your choices

You can ask what we hold about you, ask us to correct it, or ask us to delete it, by writing to help@providernotes.com. Where the information is part of a medical record, the Notice of Privacy Practices sets out rights that are broader than these and those apply instead.

7. Changes

The effective date at the top shows when this was last revised.
