Florida sets these criteria and prints them on form HSMV 83039. We do not decide them, and we cannot widen them.
What the State Asks a Practitioner to Certify
That your condition falls within one of the categories Florida lists. The most common is a severe limitation in your ability to walk because of an arthritic, neurological or orthopedic condition.
Others include conditions affecting the heart or lungs, the use of portable oxygen, legal blindness, and the temporary effects of surgery or injury. Pregnancy has its own permit and its own form.
The practitioner decides, not the form and not us. They evaluate what you tell them and what your records show, and they reach their own judgement. Nobody here is paid more for reaching one conclusion than the other.
How to Answer the Questions
Describe your situation as it actually is, on an ordinary day. Not your best day, and not your worst.
If you are unsure how to answer something, say so in the box rather than guessing. A practitioner would far rather read "it varies, some days I manage and some days I cannot" than a number you picked to be safe.
A certification obtained on inaccurate information may be void. Your county can refuse it, the practitioner can withdraw it, and knowingly providing false information on a state application carries penalties under Florida law. This is why we ask rather than tell.
Who We Cannot Evaluate
- Anybody under 18. A permit for a child goes through their own doctor.
- Anybody outside Florida at the time of the evaluation. Florida law requires you to be physically in Florida when a Florida-licensed practitioner evaluates you.
- Anybody whose condition falls outside the practitioners' scope, which we will tell you before you pay.
