Our practitioners are licensed in Florida, and a certification has to come from a practitioner licensed in the state where you are. Here is what you can do instead.
Provider Notes serves patients located in Florida. If you are in another state, we cannot complete your certification, because the practitioner has to be licensed where you are. Your own doctor can complete your state's form, and most states accept certifications from several practitioner types, not just a primary care physician.
If you hold a valid disabled parking permit from another U.S. state, it is generally honored for accessible parking in Florida. You do not need a Florida permit to visit. Local time limits and posted restrictions still apply.
