Notice of Privacy Practices

This notice describes how medical information about you may be used and disclosed, and how you can get access to this information. Please review it carefully.

Who this notice covers

Fishman Mobility Services, P.A. ("the Practice") and its licensed practitioners. Access Path Management, LLC, doing business as Provider Notes, acts as the Practice's business associate and is bound by a written agreement to protect your information on the same terms.

Our obligations

We are required by law to keep your health information private, to give you this notice, and to follow it. We must tell you if a breach affects your unsecured health information.

How we may use and disclose your information

For treatment. A practitioner reviews your answers, your identification and any records in order to evaluate your condition.
For payment. To charge your card and to keep a record of the charge.
For health care operations. Quality review, records management, and communications about the evaluation you requested.
To the person you asked us to send it to. Your completed form goes to you, or to the personal representative who requested it on your behalf.
As the law requires. Including to public health authorities and in response to lawful process.

Any other use or disclosure requires your written authorization, which you may revoke at any time.

Marketing

We do not sell your health information, and we will not use it for marketing without your written authorization. Messages about an evaluation you requested are health care operations rather than marketing and are sent regardless of any marketing preference.

Your rights

To see and copy your record. Ask and we will provide it, usually within 30 days.
To correct something you believe is wrong.
To an accounting of certain disclosures we made.
To ask us to restrict a use or disclosure. We will consider it.
To ask for confidential communication by a different means or at a different address.
To a paper copy of this notice, even if you agreed to it electronically.
To complain, to us or to the Secretary of the U.S. Department of Health and Human Services. We will not retaliate.
