Terms of Service
Effective October 28, 2026. Access Path Management, LLC, doing business as Provider Notes.
1. Who we are
Provider Notes is a service of Access Path Management, LLC, a Florida limited liability company ("the Company"). The Company arranges evaluations, provides the technology and administrative services, and is the party you pay. It does not provide medical or chiropractic care and makes no clinical decisions.
All evaluations are performed by licensed practitioners of Fishman Mobility Services, P.A., an affiliated professional practice ("the Practice"). The Practice and its practitioners are solely responsible for all clinical decisions, including whether to certify any condition. The Practice and the Company are under common ownership. The Company pays the Practice a fixed fee for each evaluation performed, which is the same whether or not a patient is certified.
2. What the Services are
The Services consist of a free eligibility check, an online evaluation request, and a telehealth evaluation by a licensed practitioner of the Practice.
The evaluation is offered in more than one way. Which are available depends on the Service State you are in, and you are shown what is available, and the fee for it, before you pay:
- An asynchronous case review, in which the practitioner evaluates your written answers, your identification and any records you provide, without a live session.
- A synchronous video visit, a short live appointment with the practitioner at a scheduled time.
Both are telehealth under the law of each Service State, performed by licensed practitioners of the Practice. In every case the evaluation is performed by a licensed practitioner exercising independent professional judgment. The Services are not an automated determination.
3. Where the Services are offered
| State | Status | What is offered |
|---|---|---|
| Florida | Open | Case review and Video visit |
| Arizona | Open | Video visit |
| Utah | Open | Video visit |
| Idaho | Open | Video visit |
| New Hampshire | Open | Video visit |
| Delaware | Open | Video visit |
| Every other state | Not open | You may ask to be told when we open |
You must be physically present in a Service State at the time of your evaluation. This is a requirement of state law and not a policy of ours.
4. What the Services do not include
- No permit is issued by us. Permits are issued by your state, directly or through a county office.
- No outcome is guaranteed. The practitioner decides whether your condition meets the criteria.
- No ongoing care. The Services are a single evaluation for a single form.
- No emergency care. If you have a medical emergency, call 911.
- No prescriptions, referrals or treatment of any kind.
5. Who may use the Services
You must be 18 or older. A person may complete a request on behalf of another where they are helping that person, who is present and consenting, or where they are acting as that person's personal representative. Where you act as a personal representative you will be asked your relationship, the basis of your authority, and to attest that you are authorized.
6. What you agree to provide
Accurate and complete information. A certification rests on what you tell the practitioner, and a certification obtained on false information may be void, may be refused by your county, and may expose you to penalties under state law.
7. Fees
In every Service State the fee is $119. The fee is paid to the Company. Your card is saved when you request an evaluation and charged in full, once, when the evaluation begins: when the practitioner begins reviewing a case review, or one hour before a video visit. See the Refund Policy.
8. Communications
We communicate by email. Messages about your evaluation are part of the Services and are sent regardless of any marketing preference. Nothing about your health is placed in the body of an email; clinical content sits behind a verification of your identity.
9. Intellectual property
The Company owns this site and its content. You may print anything here for your own use.
10. Disclaimers and limitation of liability
The Services are provided as described and without warranty beyond what the law requires and does not permit to be excluded. Nothing here limits a practitioner's professional obligations to you.
To the fullest extent the law permits, the Company's aggregate liability arising from the Services is limited to the fee you paid.
11. Governing law
These terms are governed by the law of the State of Florida.
12. Changes
We may change these terms. The effective date at the top shows when they were last revised. Changes do not apply retrospectively to an evaluation already paid for.
