Accessibility Statement
Effective October 28, 2026. Access Path Management, LLC, doing business as Provider Notes.
Why this matters here more than most
Everybody using this service has a condition that affects their mobility, and most are over 65. A site that is difficult to use is not an inconvenience for our patients; it is the same barrier they came here to have recognized.
What we have done
- Type that starts large. 17 pixels on a phone, and everything scales if you enlarge it in your browser or your phone's settings.
- Targets that can be hit. Every button and link is at least 56 pixels tall.
- Contrast. Text meets or exceeds the WCAG 2.1 AA contrast ratio.
- Keyboard. Every part of the site can be reached and used without a mouse, with a visible focus outline.
- Screen readers. Headings are in order, images have descriptions, and forms have real labels.
- No motion you did not ask for. Nothing auto-rotates, and anything that moves respects your reduced-motion setting.
- No horizontal scrolling, at any width from 320 pixels upward.
Our standard
We aim to meet WCAG 2.1 Level AA. We are not claiming certification; there is no such thing. We test at phone, tablet and desktop widths and with a keyboard, and we fix what we find.
Where we know we fall short
We have not completed a full audit with assistive technology users. When we do, this section will say what it found.
Tell us
If something here does not work for you, we want to know, and we will help you complete what you came to do in the meantime.
Call (754) 233-0586 on a weekday, 9am to 6pm Eastern, and a person will answer. Or write to help@providernotes.com. We aim to reply within two business days.
If the website is the obstacle, the telephone is not. Anything that can be done here can be done on a call, and the call is free.
