Nobody here is paid to approve you

The doctors are paid for the evaluation, and the fee does not change with the outcome. A service that pays per approval produces certifications that mean nothing, and counties notice.

It is also why the free check is written to turn people away.

Your information

  • The check saves nothing until you give a name and email at the end.
  • Your medical information lives in a HIPAA-covered health record.
  • Our emails never contain health information. They contain a link behind an identity check.
  • Every vendor that touches it has a signed business associate agreement.
  • You can ask for a copy of anything, at any time.

The structure

Provider Notes is a service of Access Path Management, LLC, which arranges evaluations, provides the technology, and is the company you pay. It does not provide medical care.

Evaluations are performed by licensed practitioners of Fishman Mobility Services, P.A., an affiliated professional practice, who are solely responsible for every clinical decision.

What this service does not do

  • We do not issue permits. Your state does. We complete the medical certification your state's form requires.
  • We do not guarantee certification. The doctor decides, on what you tell them, and is paid the same either way.
  • We are not ongoing care. This is one evaluation for one form, not a doctor you keep.
  • We do not accept insurance or Medicare. One fee, by card, charged only when the doctor begins.
  • We are not a government agency, and we are not affiliated with one.
