Provider Notes

How it works

No office visit for the evaluation, and nothing to arrange transport for.

  1. Take the free check

A few short questions about your condition and your walking, written for your state. Free, about two minutes, and nothing is saved until you decide to go on.

  2. Add a payment method

Nothing is charged yet. The fee is taken once, when the doctor begins your evaluation.

  3. Your details and a photo of your ID

Name, date of birth and address exactly as on your state ID. The state matches the two, so one letter matters.

  4. Questions for your doctor

About your condition, how far you can walk, and what treatment you have had. You can stop and come back.

  5. A doctor evaluates you

In Arizona, Utah, Idaho, New Hampshire and Delaware, in a short video visit. In Florida, as a written review or a video visit, whichever you chose.

  6. You hear back

By email after a written review, usually within two business days; at the end of a video visit, the doctor tells you. Your completed form comes to you if the doctor certifies you, by email, or by post where your state needs the original. If not, you get a written explanation of why.

  7. Your state issues the placard

The form comes with instructions for sending it in, because every state does this differently. The form itself is free from your state.

Two ways to be seen

Video visit

A short live appointment with the practitioner at a time you pick.

Offered in Florida, Arizona, Utah, Idaho, New Hampshire and Delaware.

Written review

Answer in writing, on your own time. No appointment.

Offered in Florida.

Which are available depends on your state and its law, and the check tells you before you commit to anything. Both are telehealth, performed by a practitioner licensed in your state, to the same standard and for the same fee.

Every state requires you to be physically in that state during the evaluation. That is state law, not a policy of ours.

How we reach you

By email. Nothing about your health goes in the message itself; it sits behind a check of your last name, date of birth and ZIP code.

Add updates@providernotes.com to your contacts so we do not land in spam.
