Florida Parking Permit for Pregnancy: Form HSMV 83040

Florida has a permit specifically for expectant mothers, on a different form from every other condition.

Conditions in This Group

  • Any stage of pregnancy, confirmed by a provider
  • A pregnancy your provider considers high-risk
  • A pregnancy with advised limits on walking, standing or lifting

This permit uses form HSMV 83040 rather than 83039, and Florida requires a physician to sign it. The questions are about the pregnancy rather than about walking, because that is what the form asks.

Not the whole list. The check lets you describe your condition in your own words.

What to Have on Hand

The practitioner will ask about the things below. Get the information and any documents together now to streamline the process.

  • Your due date, and how many weeks along you are
  • Whether a doctor, midwife or clinic has confirmed the pregnancy
  • Who your OB-GYN, midwife or prenatal provider is
  • When your last prenatal visit was
  • Whether your provider considers the pregnancy high-risk
  • Whether you have been advised to limit walking, standing or lifting

Answer as your situation actually is. Records help but are not required, and we can request them from your own doctor if that is easier.
