HSMV 83039 is the Florida Application for Disabled Person Parking Permit. It has two parts: a section you fill in yourself, and a Physician or Certifying Practitioner's Statement of Certification that a licensed practitioner must complete and sign within 12 months of you submitting it. You take the completed form to your county tax collector or tag agency, and they issue the placard.
The official title is Application for Disabled Person Parking Permit. People search for it as the Florida handicap form, the Florida handicap parking permit form, the Florida DMV handicap application, and the disabled person parking permit application. They are all the same document: HSMV 83039.

The certification has an expiry. The practitioner's signature must be dated within 12 months of the date you submit the application. A form signed two years ago will be rejected.
Florida does not limit this to your primary care physician. The certifying authorities include a physician licensed under Chapter 458, an osteopathic physician under Chapter 459, a chiropractic physician under Chapter 460, a podiatric physician under Chapter 461, an optometrist under Chapter 463 for vision-related certifications, an advanced practice registered nurse under Chapter 464 working under a physician protocol, and a physician assistant under Chapter 458 or 459. A similarly licensed physician from another state may also sign, provided the application includes documentation of that license and confirms their knowledge of Florida's eligibility rules.

The certification section asks the practitioner to identify which listed condition applies. The standard Florida uses is functional, not diagnostic. It is about what you can safely do, not what you have been diagnosed with. In broad terms the form covers:
Note the wording on assistive devices. If a device restores your ability to walk to the point where you can walk without severe limitation, the form says you are not eligible. The question is whether the limitation remains, not whether you own a cane.
Nobody outside the state issues a Florida placard. Any service that says otherwise is describing something that does not exist. What can be done from home is the medical certification step.
Florida law permits licensed practitioners to evaluate patients by telehealth within their scope of practice. A video visit is a real clinical evaluation, not a form-filling service: the practitioner asks how far you can walk, whether you stop to rest, what devices you use, and how your condition affects you day to day, then reaches their own conclusion.
