Florida placards are issued by county tax collectors, not by the state and not by a doctor. What to bring and what it costs.
Your County Tax Collector, Not the State
Florida's Department of Highway Safety and Motor Vehicles publishes the form and sets the criteria, but the permit itself is issued locally, at a county tax collector's office.
What to Bring
- The completed HSMV 83039, with the practitioner's half signed and dated
- Your Florida driver license or ID card
- $15 if the permit is temporary. A permanent one is free.
Counties Differ, and It Is Worth One Phone Call
Some accept the application by post or by email. Some require it in person. Some issue at the counter in minutes and some post it within a week. Their website will say, and their phone line will say faster.
What a Doctor Cannot Do
Issue the permit. A practitioner completes the medical certification; the decision to issue is the county's. Any service that tells you otherwise is describing something it does not control.
