Florida calls it the wheelchair symbol license plate. It uses the same medical certification as a permanent placard, so if you qualify for one you qualify for the other.

Florida issues a wheelchair symbol license plate to a vehicle owner or lessee who qualifies for a disabled parking permit, under section 320.0843, Florida Statutes. You qualify with either form HSMV 83007 or a completed HSMV 83039, the same form used for a placard. It is issued in place of a permanent placard, costs $28 plus your normal registration fees, and renews every year on your birthday.

The plate is issued in lieu of a permanent placard, not in addition to it. For most people a placard is the more flexible choice, and it is free. Think carefully before choosing the plate, particularly if family members ever drive you.
The plate is for people with a long-term mobility impairment, or who are certified as legally blind. The medical standard is the same one used for a permanent placard, so the practitioner is assessing the same thing: whether your condition severely limits your ability to walk.
The plate is not available for temporary conditions. If you are recovering from surgery, have a fracture, or have a pregnancy-related limitation, a temporary placard is the right route. Those are valid for up to six months and the plate is not an option.
The vehicle must be registered or leased in your name. You submit all of it to a motor vehicle service center or your county tax collector.

Wheelchair symbol plates renew annually on your birthday, alongside your normal vehicle registration. The long-term impairment is proved at the initial application. Check with your county about what, if anything, they require at each renewal, since practice varies.
