How cases are assigned
By condition and by state. In Florida, joint and back conditions go to Dr. Fishman and everything else to Dr. Schapiro. Outside Florida, every case goes to Dr. Schapiro under his license in that state.
How they are paid
The same, whether or not they certify you. That is why the answer means something, and why the free check is written to tell you early when the criteria do not fit.
