Who qualifies for a placard in Arizona
These are the criteria on Arizona’s form MVD 96-0104, in plain words. A doctor applies them; this page cannot.
- You cannot walk 200 feet without stopping to rest.
- You cannot walk without a cane, crutch, brace, prosthetic device, wheelchair or another person’s help.
- You have lung disease that severely limits your breathing.
- You use portable oxygen.
- You have a heart condition classed as Class III or IV by the American Heart Association.
- You are severely limited in your ability to walk because of an arthritic, neurological or orthopedic condition.
Arizona’s form has no box for legal blindness or loss of a limb on its own; those qualify through the walking criteria.
You may not need a doctor: A veteran rated 100% service-connected disabled can apply with a VA certificate instead of a doctor’s certification.
