A Delaware DMV branch, in person. A first placard is issued there and then. The placard is free; a replacement is $1.

What to bring

1. The signed original MV474. Delaware accepts the original only: no copies, faxes, scans or downloads.
2. Your Delaware driver license or ID card.
3. If you are doing this for someone else, ask Delaware DMV what they need from you.

Without a visit

None for a first placard.

Do not have the form yet?

It is free from Delaware Division of Motor Vehicles. Your own doctor can complete the medical part.

Mistakes that cause a second trip

• A copy, scan or fax instead of the original.
• Applying with a doctor’s certification at 80 or over, when none is needed.
