Form MV474, explained
MV474 is Delaware’s application for a disabled parking placard. It has an applicant section you complete and a medical certification a licensed practitioner completes. It is free.
Where to get it
Download it free from Delaware Division of Motor Vehicles. We link to the state’s own copy rather than hosting one, so you always have the current revision.
What the medical section needs
- Which of the state’s criteria apply, in the practitioner’s judgment.
- Whether the condition is temporary or permanent, and for a temporary placard, how long.
- The practitioner’s name, license number and signature. Delaware accepts only the original signature, not a copy, fax or scan.
Who may sign it
In Delaware, a physician, or a physician assistant or advanced practice nurse working with one.
Where the signed form goes
Take the signed original to a Delaware DMV branch in person. Delaware issues a first placard in person, and accepts only the original form, not a copy, fax or scan. None for a first placard: it is issued in person. The placard is free.
