Who qualifies for a placard in Delaware
These are the criteria on Delaware’s form MV474, in plain words. A doctor applies them; this page cannot.
- You cannot walk 200 feet without stopping to rest.
- You cannot walk without a cane, crutch, brace, prosthetic device, wheelchair or another person’s help.
- You have lung disease that severely limits your breathing.
- You use portable oxygen.
- You have a heart condition classed as Class III or IV by the American Heart Association.
- You are severely limited in your ability to walk because of an arthritic, neurological or orthopedic condition.
A long-term placard means the doctor sees no prognosis for improvement. People aged 80 and over can apply without any medical certification.
You may not need a doctor: At 80 or older you do not need a doctor’s certification at all: apply at the DMV directly.
Who may sign the medical part
In Delaware, a physician, or a physician assistant or advanced practice nurse working with one. The form is free from Delaware DMV, and your own doctor can complete it.
Temporary or permanent
A temporary placard lasts 180 days (a red placard, for 180 days. extending it is a new application.) and the state charges free. A permanent placard is valid eight years (a blue placard, for eight years. renewals are self-certified.) and the state charges free.
