Already have a signed form? Here is where it goes

Your county tax collector or license plate agency, in person. Find yours on the FLHSMV site. The placard is free for a permanent placard; $15 for a temporary one.

What to bring

  1. The signed HSMV 83039. The doctor’s section must be complete and signed, and the signature must be less than 12 months old.
  2. Your Florida driver license or ID card.
  3. If you are doing this for someone else, ask FLHSMV what they need from you.

Without a visit

Some counties accept the form by mail; call your county to ask.

Do not have the form yet?

It is free from Florida Department of Highway Safety and Motor Vehicles. Your own doctor can complete the medical part.

Mistakes that cause a second trip

  • Your own signature is missing from the applicant section.
  • The doctor’s license number is missing or unreadable.
  • The form is more than 12 months old.
  • The wrong box (temporary or permanent) is checked.
