Who qualifies for a placard in New Hampshire
These are the criteria on New Hampshire’s form RDMV 130, in plain words. A doctor applies them; this page cannot.
- You cannot walk without help or a cane, crutch, brace, prosthetic device or wheelchair.
- You have lung disease that severely limits your breathing, or you use portable oxygen.
- You have a heart condition classed as Class III or IV by the American Heart Association.
- You are severely limited in your ability to walk because of an arthritic, neurological or orthopedic condition.
- You have another medically debilitating condition that limits your walking, which the doctor describes on the form.
New Hampshire no longer uses the 200-foot test. The doctor describes your condition, how it limits your walking, and the outlook, in writing on the form.
Who may sign the medical part
In New Hampshire, a physician, podiatrist, advanced practice registered nurse or physician assistant licensed in New Hampshire. The form is free from NH DMV, and your own doctor can complete it.
