Already have a signed form? Here is where it goes

A Utah DMV office, or by mail to Utah DMV, PO Box 30412, Salt Lake City, UT 84130. The placard is free for a paper placard; $2.50 for a durable one.

What to bring

1. The signed TC-842. The certifier must be licensed in Utah.
2. Your Utah driver license or ID card.
3. If you are doing this for someone else, ask Utah DMV what they need from you.

Without a visit

The form can be mailed.

Do not have the form yet?

It is free from Utah State Tax Commission, Division of Motor Vehicles. Your own doctor can complete the medical part.

Mistakes that cause a second trip

• A certifier who is not licensed in Utah.
• Asking to renew a temporary placard: a new certification is needed instead.
