Form TC-842, explained

TC-842 is Utah’s application for a disabled parking placard. It has an applicant section you complete and a medical certification a licensed practitioner completes. It is free.

Where to get it

Download it free from Utah State Tax Commission, Division of Motor Vehicles. We link to the state’s own copy rather than hosting one, so you always have the current revision.

What the medical section needs

  • Which of the state’s criteria apply, in the practitioner’s judgment.
  • Whether the condition is temporary or permanent, and for a temporary placard, how long.
  • The practitioner’s name, license number and signature.

Who may sign it

In Utah, a Utah-licensed physician, physician assistant, nurse practitioner or physical therapist.

Where the signed form goes

Take the signed form to a Utah DMV office, or mail it to Utah DMV, PO Box 30412, Salt Lake City, UT 84130. The form can be mailed. The placard is free (a durable placard is $2.50).
