Who qualifies for a placard in Utah

These are the criteria on Utah’s form TC-842, in plain words. A doctor applies them; this page cannot.

  • You cannot walk 200 feet without stopping to rest.
  • You cannot walk without a cane, crutch, brace, prosthetic device, wheelchair or another person’s help.
  • You have lung disease that severely limits your breathing.
  • You use portable oxygen.
  • You have a heart condition classed as Class III or IV by the American Heart Association.
  • You are severely limited in your ability to walk because of an arthritic, neurological or orthopedic condition.

Utah’s form has no separate box for blindness or loss of a limb. A wheelchair-user placard is your choice when you apply.
