Common questions

Good to know

Is this the DMV?

No. We’re a private medical service. A Florida-licensed doctor fills in the medical part of form HSMV 83039. FLHSMV issues your placard.

How much is it, and when am I charged?

The check is free. The doctor’s review is $119. You add a payment method, and it’s charged when your doctor begins: when they start a written review, or one hour before a video call.

Do I need my surgeon’s notes?

No, but they help. Add them if you have them.

Video call or written review?

You choose. A written review needs nothing scheduled. A video call takes 5 to 10 minutes at a time you pick. You need to be in Florida either way. Family can help.

How fast is it?

A written review usually finishes within two business days. Video calls are often open within a day or two.

What if the doctor says no?

You’ll get a short written note explaining why. The $119 pays for the doctor’s time to evaluate you, so it isn’t refunded. See our Refund Policy.
