Replacement of a Florida disabled parking permit is handled by your county tax collector or tag agency. Florida publishes form HSMV 83146, Application for Replacement License Plate, Validation Decal or Parking Permit, for this purpose. Whether a new medical certification is needed depends on your situation and your county, so contact them first.
This is the part most people get backwards. A replacement is an administrative process, not a clinical one. Your permit already exists in the county's records. Contact your county tax collector or tag agency first and ask what they need, before arranging any appointment.
If your permit was near the end of its validity anyway, some counties will treat the request as a renewal rather than a replacement, which does require a current HSMV 83039 signed within the last 12 months. That is the one situation where a replacement turns into a certification appointment.
Ask your county the direct question: "Do you need a new medical certification for this replacement, or just the replacement application?" That one question saves an unnecessary appointment.
Report it. A disabled parking permit is a credential tied to you personally, and a stolen one can be misused in your name. Your county may ask whether it was reported.
You do not have a valid permit until the replacement is issued. Parking in an accessible space without a valid displayed permit carries a fine and the vehicle can be towed, regardless of the circumstances.
